  • Jersey Assistance for Community Caregiving Amendment (modified: 1 year ago)
    New Jersey requests to amend the New Jersey FamilyCare demonstration to add the Jersey Assistance for Community Caregiving (JACC) population into the demonstration. The JACC program will provide a broad array of in-home support services to enable an individual at risk for nursing facility placement to remain in their home thus delaying placement in a nursing facility. CMS originally posted this request for a 30-day federal public comment period from October 20, 2017 to November 20, 2017 but recently learned that some application documents submitted by the state were missing. To ensure the public has opportunity to provide input on the state's complete application, CMS is providing a second federal public comment period from April 2, 2018 to May 2, 2018.

    New Jersey is proposing to amend its current 1115 demonstration, New Jersey Comprehensive Waiver, as it pertains to the Managed Long Term Services and Supports (MLTSS) Program. The proposed amendment will better define the level of care (LOC) requirements in the Special Terms and Conditions for adults and children seeking enrollment in MLTSS. In the current demonstration, the level of care requirement allowed for an adult's definition of Nursing Facility LOC is used for a child, thus enabling children who may not be appropriate for MLTSS services to enter the program. New Jersey has proposed clarifications to the enrollment and nursing care criteria for eligible children within the amendment. The federal public comment period is open from December 1, 2016 through January 2, 2017.

    New Jersey requests to amend its demonstration to create two new eligibility groups for the Supports Program and allow individuals in the Supports Program who are eligible for and in need of Private Duty Nursing services to access those services through Managed Long-Term Services and Supports program. The federal public comment period will be open from August 10, 2015 through September 9, 2015.

    New Jersey's extension request seeks to extend many elements of the current demonstration and adopt new approaches for treatment of behavioral health and substance use disorders, integrated care for incarcerated individuals, expansion of population health partnerships, and development of housing support services. The federal public comment period will be open from March 10, 2017 through April 10, 2017.

    New Jersey requests to amend the New Jersey FamilyCare demonstration to expedite financial eligibility determinations, add a one-time allowance for pantry stocking and clothing to the community transition benefit, and implement the New Jersey Home Visiting pilot program for high-risk pregnant/postpartum women, infants and young children up to age two. The federal public comment period will be open from September 7, 2018 to October 7, 2018.
